I didn’t pass the eligibility quiz. Why not?

The quiz questions are designed to make sure that an organization is eligible per Nationwide Insurance Foundation's funding guidelines. If you received a message that your proposal doesn’t meet the funding guidelines, the Foundation regrets that we won’t be able to provide support to your organization. If you have questions about our funding guidelines please send them to corpcit@nationwide.com.

After saving my application, how do I return to review, edit and submit the funding application?

To save your application and complete it at a later time, select Save & Finish Later at the bottom of the page. You'll be asked to enter an email address and password.

You'll then receive an automated confirmation email providing you with a link to use to return to the application and your My Account page.

Can I return to the original link to access the application?

No, accessing that link will start a new application. You must log in to the My Account page to continue the original application.

I’ve entered my email address and password and still can’t access my application or My Account page.

Please double check your entries to make sure that you have not misspelled anything and that you’re using the same email address that you used to create your account. If your organization has previously applied, you must use the same email address used to create your account. If you still have problems, contact corpcit@nationwide.com.

 I’ve logged into my account and my application isn’t listed.

If your application is not found, it’s most likely that the application wasn’t saved properly and you’ll need to start a new application.

I forgot my password. What do I do?

Go to www.nationwide.com/foundation. Select 'Ready to Apply,' then select 'Complete a previously saved application.' On the login page, select the "Forgot your password?" link. You'll receive an email, to the email address with the password used to submit or save the application.
