Accident medical insurance safeguards your customers’ sponsored group or program if an accidental injury or death occurs while the insured group is participating in their group activities or events.
Group accident medical benefits:

Apply for coverage
To apply for coverage, just print and complete an application. Mail the application along with a check for first premium payment to the address listed on the application.
Clients who prefer to pay premium via electronic deduction from a checking or savings account may print an ACH check by phone form and either mail or fax it along with the application for coverage.
