What information should you provide when reporting a claim?

• Policy number or insured name
• Location of loss (city, state and county)
• Names and phone numbers of persons involved
• Description of injuries
• If you’re reporting an auto accident claim, include vehicle make, model, year and VIN number
• If you’re reporting a workers compensation claim, you may be required to file a state-specific First Report of Injury
