• Primary (first-to-pay) and excess (next-to-pay) plan options

• Accidental death and specific loss (loss of limb, sight, hearing) benefits
• Coverage for out-of-pocket expenses like deductibles, co-insurance and prescriptions 

Ready to apply?

Just print and complete an application for coverage. Mail the application with a check for first premium payment to the address listed on the application. Clients who prefer to pay premium via electronic deduction from a checking or savings account may print an ACH check by phone form and either mail or fax it along with the application to begin the policy issue process.
