How to nominate an individual for a CARE Award
Select the nomination form below to recommend an outstanding leader in a youth-serving organization. The nomination period is Nov. 12 through Dec. 19, 2025.
CARE Awards will be given out to 3 honorees per state:
1 grand prize winner per state will receive $500 and an engraved crystal trophy
2 runners-up per state will each receive $250 and an engraved plaque
