Use this guide to help lead a discussion on the important steps to take when an incident happens at work. Knowing these procedures can help workers feel more confident in promptly reporting, documenting and handling these events effectively.

1. Report the incident immediately

  • Inform your supervisor right away to gather facts and witness information before details are forgotten.
  • Prompt reporting helps address hazards, prevent future accidents, and avoid delays in resolution.
  • Report all incidents immediately, even if no treatment is needed, to document facts and avoid increased costs or delays.

2. In the event of an injury, your supervisor will assess the situation and determine the next steps

  • Serious and/or life-threatening injury (e.g. fall from heights, laceration causing significant loss of blood)
    • Call 911 immediately
    • Report the injury to Nationwide claims at 1-800-421-3535
  • Non-life-threatening injury that clearly requires treatment (e.g. broken bone, laceration requiring stitches, concussion)
    • Refer the employee an authorized treatment provider (if state allows)
    • Report the injury to Nationwide claims at 1-800-421-3535
  • Minor injury, unsure if medical treatment is needed
    • Call the Nurse Triage Hotline at 1-855-777-7090 for guidance

3. Report the claim to Nationwide

  • Your supervisor/manager will notify the designated return-to-work program administrator, who will report the injury using one of these options:

  • Nationwide Claims 1-800-421-3535 (If not using the Nurse Triage Hotline)
  • Email enewloss@nationwide.com
  • Online at nationwide.com

4. Complete an incident report

  • Supervisors should complete an incident report and any other relevant internal documentation for investigation purposes.
  • Investigate major accidents as soon as possible, injury accidents after medical treatment, and  non-injury accidents immediately.

5. What happens next

  • Nationwide will contact your company to gather information, assign a claims associate and share initial paperwork with the injured employee.
  • The injured employee must complete authorization forms as requested by Nationwide.
  • The program administrator will explore temporary modified positions and discuss next steps in the return-to-work process
