[4] Core Value Connect is available in Atlanta, Georgia; Chicago, Illinois; Columbus, Ohio; Dallas and Houston, Texas; Miami/Ft. Lauderdale, Florida; and Portland, Oregon. Pediatric and family care offerings vary by location and state. Visit fees may be applicable on HSA-eligible plans until the member deductible has been met. One Medical DPC membership includes access to offices nationwide. When visiting One Medical offices in NC, NJ and MD, additional member fees will apply. Additional fees may also apply for any nonstandard lab or services not included in the contracted DPC services agreement. The retainer medical practice/DPC services provided by One Medical in accordance with the agreement with Nationwide do not constitute comprehensive insurance coverage on its own. Employees should continue to maintain insurance for non-DPC services. If a Plan member requires services outside of One Medical, the dedicated member support team, Member Advocacy Program (MAP), is available to help navigate their plan as needed. Pharmacy benefits and transplants still rely on the use of network providers. Amazon One Medical and all related logos are trademarks of Amazon.com Inc. or its affiliates.
The Self-Funded Program through Nationwide provides tools for employers owning small to midsize businesses to establish a self-funded health benefit plan for their employees. The benefit plan is established by the employer and is not an insurance product. Stop loss insurance policies are underwritten or reinsured by Nationwide Life and Benefits Insurance Company – see nationwide.com/stoplossdisclaimer for details. Product availability and specific provisions may vary by state.
